• Individual Contracting Packet
  Adobe Acrobat PDF (Quick download)
• Corporation Contracting Packet
  Adobe Acrobat PDF (Quick download)
• Copies of License(s) for all states in which agent wishes to be appointed
• Copy of current E&O declaration page
• Check made payable to Aviva Life and Annuity Company for any non-resident appointments
